You may browse our Site without submitting any personal information to us. However, if you choose to interact with our online services, we may collect data in two ways.
First, we may collect personal data that you voluntarily submit to us. Our Site hosts secure online forms for Billing, Medical Records requests, Physician Opportunities, and Scheduling (including OB/GYN, Family Medicine, Internal Medicine, Imaging, Sports Medicine, Pediatrics). Through these forms, we may collect your first and last name, email address, phone number with area code, and your specific comments or questions. We may use this data to fulfill your request, schedule appointments, and communicate clinical updates. This information is protected and accessed only by authorized employees.
Second, we may automatically collect usage data regarding how our website is accessed. This includes your IP address, browser type, operating system, device types and IDs, network connection information, date and time stamps, specific page views and interactions, and referring URLs (the website that you visited just before coming to ours or that you visit right after ours). We may use this aggregate data to ensure our Site works properly, analyze demographic traffic trends, and optimize our digital performance. We may collect this information ourselves or through third parties, as further described below.

Google Ads: This is an online advertising service that utilizes conversion tracking cookies. It helps us measure the effectiveness of our digital outreach campaigns by monitoring when an online ad leads a visitor to perform a valuable action on our website, such as completing a scheduling form.

- Text Messaging (SMS) Communication and FCC Compliance
If you explicitly choose to provide your mobile number through our online forms to receive automated text alerts from Premier (such as appointment reminders, prescription notifications, or billing updates), you are agreeing to this policy, and the following federal regulations apply:
Prior Express Consent: By opting in, you authorize Premier to deliver automated, recurring informational, operational, and transactional text messages to your mobile number. Consent is entirely voluntary and is never a condition of receiving medical care or purchasing any services.
Flexible Opt-Out: You can cancel Premier SMS alerts at any time by replying STOP to any text message you receive from us. Per FCC guidelines, we will also honor any other clear, reasonable verbal or written requests to opt out, such as informing a patient services representative through any of the contact methods specified below.
Mobile Data Privacy & Carrier Non-Sharing: Premier highly values your personal privacy. No mobile phone numbers, SMS opt-in consent data, or personal information collected specifically for text messaging verification will be shared, sold, rented, leased, or exchanged with third parties, partners, or affiliates for marketing or promotional purposes.
Charges and Carriers
Message and data rates may apply. Please consult your service agreement with your wireless carrier, or contact your wireless carrier, to determine your phone’s pricing plan and the charges for sending and receiving SMS alerts. You acknowledge that you are responsible for any message, data, or other charges incurred as a result of using this service. Premier is not responsible for any undue delays, failure of delivery, or errors in messages.
- Data Security, Sharing Restrictions, and Rights
