You can access your medical records through our Patient Portal. There is no cost for this service.
If you require more detailed records, you can submit a medical records request online through our partner, Sharecare. Fees may apply.
Patient Online Access Form
This is exclusively for patients and legal guardians with legal paperwork requesting a copy of their medical records.
Requester Submission Portal
This option is for healthcare providers, attorneys, or if you are requesting the medical records of someone other than yourself and are not the legal guardian.
Disability FMLA Request Submission
This is for patients, disability companies, or employers to submit their FMLA or Disability form to Sharecare for completion.
